According to doctors, Tereshin suffers from a severe, aggressive infection in his arms, requiring multiple skin grafts to save the limbs. However, numerous test results indicate his health is too fragile for surgery.
After years of injecting synthol – a mixture of oil, lidocaine, and alcohol – to inflate his muscles, Tereshin's arm muscles developed severe fibrosis, leading to tissue necrosis. Videos he posted show parts of his muscle torn open, forming deep holes with decaying tissue, an image that horrified netizens.
Tereshin began injecting synthol in 2017, despite many warnings about its dangers. The excessive size of his arms led to comparisons with the cartoon character Popeye.

The Russian bodybuilder underwent multiple medical interventions to save his arms, but severe infected wounds caused recovery efforts to fail.
In 2019, activist Alana Mamaeva persuaded Tereshin to seek emergency treatment. Following a surgery, Tereshin posted a video showcasing tissue lumps he claimed were removed from his arms, calling them "treasures".
Doctor Dmitry Melnikov, who treated Tereshin at Sechenov Moscow Medical University, stated he had to remove kilograms of scar tissue and dense oil from the patient's arm muscles. Holding a 1,3 kg mass of removed tissue, he said, "This is scar tissue mixed with destroyed muscle".

In another video, Tereshin asked the doctor, "How much muscle have I lost?". Melnikov had warned him that if the toxic oil was not removed, his life could be in danger.
To date, the doctor says he has only addressed about 25% of the damaged tissue.
The current infection is progressing too rapidly, leading doctors to worry they will not have enough time to stabilize the patient's health before further surgery. If intervention is not timely, the last resort may be amputation to avoid life-threatening danger.
