A 17-year-old boy named Nam experienced significant hair loss, developing a large bald spot on the back of his head. He tried various topical medications and natural remedies like coconut oil and soapberry for about a year, but saw no improvement. Dr. Dang Thi Ngoc Bich, Head of the Dermatology - Cosmetic Dermatology Department at Tam Anh General Hospital in Ho Chi Minh City, examined Nam and found a bald spot approximately 4 cm in diameter, surrounded by hairs resembling exclamation points.
Patchy hair loss typically occurs before the age of 40, primarily affecting those between 30 and 34. Causes can include genetics, lifestyle, environment, and prolonged stress. Infections, medications, thyroid issues, and autoimmune diseases can also contribute.
"Teenage hair loss like Nam's is rare," Dr. Bich said, suggesting that stress from academic pressure may have disrupted his immune system, causing it to attack hair follicles. When triggered, the immune system can mistakenly identify hair follicles as foreign bodies and attack them, leading to patchy hair loss.

Nam received triamcinolone injections directly into the dermis of his scalp, undergoing a treatment course of 4-6 sessions spaced one month apart. The injections aimed to suppress inflammation, prevent further immune system attacks on the hair follicles, and stimulate regrowth. After 3 sessions, Nam's hair loss improved significantly, new hair began to grow, and the itching and discomfort on his scalp subsided.
According to Dr. Bich, besides triamcinolone injections, mild cases of patchy hair loss can be treated with topical immunosuppressants or minoxidil to stimulate hair growth. For more extensive hair loss, specialized therapies like laser treatment with appropriate wavelengths can promote new hair growth. Light therapy, electrophoresis, and platelet-rich plasma (PRP) injections can improve scalp circulation, heal the scalp and follicles, regenerate follicles, and stimulate hair growth.
