Linh first noticed a scattering of freckles across her forehead, cheeks, and chin after the birth of her first child 5 years ago. The condition worsened after her second child. Doctor Quach Thi Bich Van, a dermatologist at Tam Anh General Hospital in Ho Chi Minh City, examined Linh's skin and found a high concentration of melanin in both the epidermis and dermis. Dark brown spots clustered on her cheekbones, forehead, and nose bridge, merging into larger patches and interspersed with areas of melasma, resulting in uneven skin tone.

Doctor Van explained that postpartum freckling is a common pigmentation disorder in women, primarily due to hormonal fluctuations during pregnancy. Elevated estrogen and progesterone levels stimulate melanocytes to produce more melanin, the pigment responsible for skin and hair color. Uneven melanin distribution leads to the appearance of brown spots like freckles and melasma.
While hormone levels decrease after childbirth, the existing pigment doesn't disappear. UV exposure, stress, and lack of sleep can exacerbate the issue. Genetics also play a role; if a mother or grandmother had freckles, the daughter's risk is significantly higher.
Doctor Van developed a multi-pronged treatment plan for Linh. Initially, she was prescribed topical medication to inhibit melanin production and lighten the spots. This was combined with oral supplements of vitamins C and E, and glutathione to provide antioxidant protection against UV damage and free radicals. Once her skin stabilized, Linh underwent pico laser treatment, which breaks down pigment particles into tiny fragments for natural removal. This treatment also stimulates collagen production, improving skin brightness and smoothness. Compared to older laser technologies, pico lasers minimize heat damage to surrounding tissue, reducing the risk of burns, scars, or post-inflammatory hyperpigmentation.
After over a year of treatment, Linh's freckles have faded considerably, and her cheek area has a more even tone. Her skin is brighter, more radiant, and smoother, with smaller pores. Doctor Van advised her to maintain a skincare routine, use sun protection, and attend regular check-ups to maintain the results and minimize the risk of recurrence.

According to Doctor Van, pregnancy or postpartum freckles often appear quickly, are dark in color, and sometimes accompanied by melasma or darkening in areas like the armpits, abdomen, and groin. With proper sun protection and skincare, these spots may fade over time.
Treating pregnancy or postpartum freckles requires a combination of methods and a phased approach. Daily use of broad-spectrum sunscreen is crucial, along with protective measures like hats, face masks, and sunglasses when outdoors. Gentle skincare with a mild cleanser and adequate moisturization are essential. Topical products and supplements containing antioxidants like vitamin C and niacinamide can help limit new pigmentation.
After breastfeeding or with a doctor's approval, mothers can consider more intensive treatments like laser therapy, chemical peels, or prescription topical medications. Maintaining a healthy lifestyle with sufficient sleep, stress reduction, and a diet rich in vitamins and minerals supports skin health from within.
