Tuan's semen analysis at the Infertility Support Center, Tam Anh General Hospital Ho Chi Minh City (IVF Tam Anh Ho Chi Minh City), revealed 90% immotile sperm. Dr. Duong Quang Huy, Head of the Andrology Unit, diagnosed him with a large inguinal hernia and a swollen right scrotum.
Inguinal hernia is a congenital defect where the processus vaginalis fails to close completely. This allows the omentum and internal organs to descend, compressing the scrotal area. The condition constricts blood vessels supplying the testicles, leading to reduced sperm production and poor sperm quality, often due to increased DNA fragmentation. If the herniated organs cannot return to the abdominal cavity, it may cause edema, pain in the genital area, and prolonged lack of blood supply, risking necrosis and infection.
Dr. Huy and his surgical team performed an operation to dissect and return the herniated mass to the abdominal cavity. They then closed the processus vaginalis and reinforced the inguinal wall. Tuan was discharged the same day and resumed normal activities.

Six months after surgery, Tuan's follow-up ultrasound of the genital area and hormone tests confirmed recovery. A subsequent semen analysis showed his sperm count had doubled, and DNA fragmentation decreased to 10%, meeting the average standard with good motility. Tuan and his wife were advised to engage in regular sexual intercourse for natural conception.
According to Dr. Huy, inguinal hernia significantly impacts daily life and reduces male reproductive health. It can lead to self-consciousness, reluctance to intimacy with partners, and an increased risk of serious complications. Men should undergo pre-marital reproductive health check-ups or seek early treatment if they notice abnormal signs in the genital area, such as a lump, pain, or heaviness in the scrotum. If necessary, sperm cryopreservation can help preserve fertility.
