Three-phase treatment strategy helps young man reduce acne scars

An, 25, whose face was covered with pitted acne scars and dark spots, underwent a three-phase treatment strategy using IPL technology combined with RF microneedling to reduce scarring.

Dr. Dang Thi Ngoc Bich, head of the Dermatology - Cosmetic Dermatology Department at Tam Anh General Hospital in Ho Chi Minh City, examined An's skin and noted multiple pustules, inflammatory acne, and comedones on his forehead and cheeks, along with enlarged pores and numerous atrophic scars. He also had post-inflammatory hyperpigmentation, a complication of improper skincare and acne treatment.

According to Dr. Bich, the patient's prolonged inflammatory acne and scarring were severe, requiring a multi-phase treatment approach. The first phase focused on controlling inflammation, a crucial step in reducing redness and swelling, preventing complications, and further skin damage. The doctor prescribed oral anti-inflammatory and antibiotic medications, along with topical treatments to reduce acne, control oil production, and improve skin tone. Concurrently, the patient received intense pulsed light (IPL) therapy to eliminate acne-causing bacteria, reduce inflammation, brighten the skin, and fade dark spots. After about 9 months, the acne was effectively controlled, An's skin stabilized, and there were no further inflammatory relapses.

At this point, the doctor moved to the scar treatment phase. For the deep pitted scars, particularly on the cheeks, the doctor performed subcision to break down the long-standing fibrous scar tissue. Following this, RF microneedling was used to restructure the skin's surface, stimulate collagen production, and improve overall smoothness.

RF microneedling combines high-frequency radio waves with microneedling, delivering heat deep into the dermis. This stimulates the body's production of collagen and elastin, two essential components for maintaining skin elasticity, even tone, and firmness. For individuals with pitted acne scars, this technology significantly improves the skin's surface, fills in scars, and enhances overall skin quality.

Dr. Bich added that compared to traditional methods, RF microneedling offers advantages such as less pain, minimal downtime, and faster recovery. However, to achieve optimal results, patients need proper post-treatment skincare, including avoiding direct sun exposure, applying sunscreen with an SPF of 30 or higher, gentle cleansing, and refraining from using products with strong active ingredients like AHA, BHA, or retinol in the initial days. Maintaining hydration and drinking plenty of water also promote skin regeneration. Regular follow-up appointments ensure safe and sustainable skin recovery.

After over a year of treatment, An's skin showed remarkable improvement, with faded acne scars, a more even skin tone, and increased smoothness.

Dr. Bich noted that many people mistakenly believe that after acne clears, the skin will naturally recover. In reality, pitted scars result from deep damage to the skin's structure. Without proper intervention through specialized methods like RF microneedling, laser treatments, or mesotherapy, significant improvement is unlikely.

Acne scar treatment requires time and a personalized approach tailored to each individual's skin. People should avoid home remedies like applying fresh turmeric, lemon, or honey, or using various cosmetic products, as these are often ineffective and can increase skin sensitivity, irritation, darkening, and prolong recovery time.
