The accident was caused by a girl, seated on an adult's lap in the rear seat, opening the car door. The collision involved 6 vehicles and occurred on 6/2 on Jalan Tun Razak, Kuala Lumpur.

Police stated that the child safety lock was not engaged, allowing the door to be opened from the inside. Police are investigating the incident under Malaysia's current road traffic laws for careless and reckless driving.
