A child after five years of unexplained infertility

Ms. Ngoc Hon, 35, had normal reproductive health check-ups but could not conceive for five years. She gave birth to a healthy child after in vitro fertilization.

Tests at the Center for Reproductive Assistance, Tam Anh General Hospital, TP HCM (IVF Tam Anh TP HCM), showed Ms. Hon's ovarian reserve was stable, fallopian tubes patent, and uterus normal. Her husband, Mr. Thanh, 34, had average sperm density and good motility. Doctors concluded they had unexplained infertility.

They decided to pursue in vitro fertilization (IVF) to have a child. Ms. Hon underwent ovarian stimulation; doctors retrieved 11 oocytes and created six day 5 embryos. The first embryo transfer resulted in an ectopic pregnancy in the right fallopian tube, despite the embryo being placed in a favorable uterine position with good endometrial lining. The next embryo transfer also failed.

Dr. Nguyen Thi Thuy advised Ms. Hon to have a hysteroscopy, and the results were normal. However, histopathology from a biopsy sample revealed an abnormal increase in plasma cells, diagnosing chronic endometritis. This likely caused the previous embryo transfer failures. Doctors prescribed antibiotics for two weeks to treat the uterine inflammation, then monitored her endometrial lining for a third embryo transfer.

Ms. Hon became pregnant and delivered via cesarean section at 40 weeks of pregnancy. Baby Bap was born in 3/2026, weighing 3,2 kg.

According to Dr. Thuy, unexplained infertility accounts for 15-30% of infertility cases where comprehensive reproductive health checks reveal no abnormalities. At IVF Tam Anh TP HCM, this condition affects 30-40% of couples seeking treatment. Many initially monitor for natural conception, missing the optimal treatment window. If a couple has been trying to conceive for over four years, the chance of natural conception drops to under 1%.

Doctors advise couples who haven't conceived after one year of marriage (or six months for women over 35 years old) to seek early examination and treatment. At IVF Tam Anh, treatment protocols for unexplained infertility are individualized, from lifestyle adjustments to specialized interventions, to optimize success rates. The artificial intelligence (AI)-powered embryo culture system helps select embryos with the highest implantation potential. For cases with multiple failed embryo transfers, preimplantation genetic testing for aneuploidy (PGT-A) combined with hysteroscopy helps detect and manage abnormalities, increasing pregnancy rates and healthy live births.
