Doctors remove two iron rings to save man's genitals

A 38-year-old man from An Giang wore two iron nuts, each about 3 cm in diameter, on his penis, causing deformation, swelling, and pain.

The patient arrived at Can Tho Central General Hospital with a deformed penis, constricted by two nuts. Doctors successfully removed them using a cutter after other methods failed. The man is now stable and expected to be discharged in a few days.

The two hexagonal nuts, 3 cm in diameter and 1 cm thick, had constricted the man's penis for about one month. One nut was positioned 1 cm from the base, the other 5 cm away, leading to ulceration, bleeding, and necrosis.

The medical team, recognizing the size, rigidity, and prolonged constriction, determined that conventional removal failed. They had to resort to using a cutter to carefully saw through the two iron rings.

Dr. Truong Cong Thanh, Head of the Department of Renal and Urinary Surgery, who led the procedure, emphasized that genital constriction by foreign objects is a dangerous surgical emergency. Such objects can obstruct circulation, leading to swelling, bruising, and tissue necrosis.

Untreated, this condition carries risks of serious complications including widespread damage and life-threatening consequences. Patients experiencing this must seek immediate medical attention. Early intervention allows for simpler removal with local anesthetic, but delayed cases involving large or tightly constricted metal objects require surgical intervention with specialized tools.

In severe instances where the penis has undergone necrosis with severe infection and the constriction cannot be relieved, amputation may be necessary. Doctors strongly advise against using hard metal objects like nuts on the penis for any purpose, as their non-elastic nature poses risks of constriction during erection or swelling, potentially impacting reproductive health.

The practice of wearing foreign objects on genitals, sometimes done by men "to increase sensation," carries dangers.
