Cong's weight increased from 75 kg to nearly 100 kg over two years, leading to declining health and fatigue. Determined to lose weight, he initially tried a strict diet, completely eliminating carbohydrates, sugar, and fat. This approach failed after more than one week. He then switched to a 16:8 intermittent fasting regimen, eating a maximum of two meals a day, sometimes only one. After one month, he lost 2 kg but felt exhausted. Returning to normal eating, he gained 2 kg in one week, and subsequent weight loss methods also proved unsuccessful.
Cong sought treatment at Tam Anh General Hospital in Ho Chi Minh City, weighing 98 kg with a body mass index (BMI) of 35,6. Tests and an ultrasound revealed he had stage three fatty liver, high blood fat, prediabetes, elevated uric acid, and sleep apnea syndrome.

Dr. Tran Huu Thanh Tung, from the Department of Endocrinology - Diabetes, explained that Cong suffered from severe obesity, with fatty liver and blood. Visceral fat was compressing his trachea, causing short, sudden episodes of sleep apnea during sleep. The significant weight on his legs placed great pressure on his knees, resulting in pain and increasing his risk of osteoarthritis, arthritis, and varicose veins.
Doctor Tung prescribed a combination of oral and injectable medications for Cong to promote satiety and reduce cravings. A nutritionist provided specific guidance on diet and daily food intake. Additionally, an exercise specialist instructed him on low-intensity workouts, such as light weights and walking. After a one-month follow-up, Cong's weight dropped from 98 kg to 91 kg. After nearly three months, he had lost a total of 16 kg, now weighing 82 kg. His health stabilized, and indicators for liver fat, blood fat, and uric acid improved. He experienced reduced snoring and no longer felt exhausted or fatigued.

Obesity, dyslipidemia, diabetes, and increased leptin production also contribute to osteoporosis, muscle and bone weakness, leading to muscle atrophy and an increased risk of fractures from falls. Doctor Tung advised overweight individuals to lose 5-10% of their body weight to lessen pressure on the musculoskeletal system and protect overall health.
