Drinking lemon water to treat gout?

Many people circulate the idea that drinking fresh lemon water helps reduce uric acid, potentially replacing gout medication. Is this true? (Tuan, 32, Hanoi).

Gout arises from persistently high urate levels in the blood, leading to crystal formation in joints, soft tissues, or kidneys, which causes severe inflammatory attacks. Effective gout treatment focuses on maintaining sufficiently low blood uric acid levels over an extended period to prevent new crystal formation and dissolve existing ones, rather than relying on temporary dietary remedies.

Lemons contain vitamin C, which can mildly increase urate excretion through the kidneys. This has led many to believe that lemon water benefits individuals with elevated uric acid. For some, the citrate in lemons may also alter urine composition, reducing the risk of uric acid stone formation. Thus, lemon is sometimes suggested as a supportive measure for urinary tract conditions.

While lemon water may offer some support in managing uric acid, primarily due to its vitamin C and citrate, its effect is mild and cannot replace prescribed gout medication. Patients experiencing recurrent arthritis attacks, tophi development, joint damage, or kidney complications require specific drugs to bring uric acid levels to the target range.

Patients with gout should avoid mixing lemon water with excessive sugar or honey. Individuals with gastritis, gastroesophageal reflux disease (GERD), sensitive tooth enamel, or specific kidney conditions should also avoid over-consuming lemon water. The safest and most effective strategy for gout management involves maintaining a healthy lifestyle, adhering to a balanced diet, ensuring adequate hydration, and consistently taking prescribed medication.
