Dr. Pham Thi Thu Ha of the Weight Control and Obesity Treatment Center at Tam Anh General Hospital in Ho Chi Minh City, noted Dung's body mass index (BMI) at 40.8 (kg/m2), classifying him as severely obese, with visceral fat measuring 225.2 cm2, double the normal limit. He is currently undergoing treatment for cirrhosis, stage two chronic kidney disease, high uric acid levels, and drug-induced adrenal insufficiency.
Dung's obesity increases his glomerular filtration rate, leading to chronic kidney disease and potentially end-stage renal failure, Dr. Ha explained. She warned that his enlarged fatty liver and cirrhosis could develop into inflammation or liver cancer. His weight puts pressure on his muscles, knee joints, and spine, increasing his risk of osteoarthritis. As joint inflammation or deformity progresses and activity decreases, his weight further exacerbates the pressure on his joints. Therefore, weight loss is crucial for managing his underlying conditions.
Dr. Ha recommended a personalized multi-modal weight loss program, combining medication with a balanced diet and daily exercise. A nutritionist designed Dung's meals, prioritizing protein-rich foods like chicken, fish, and eggs, along with vegetables, fruits, and nuts, while avoiding fatty foods, processed meats, and sugary items.
After a week of diligently following the diet and exercise plan, Dung lost 3 kg. After 5 weeks, he lost 9 kg and experienced improved sleep, though he still carries significant fat around his neck and abdomen. His goal is to lose 40 kg.
