Weight gain after a keto diet

Kien, 45, followed a keto diet for over two years, losing about 3 kg in the first six months before gaining over 20 kg.

Upon examination at Tam Anh General Hospital, Ho Chi Minh City, Kien weighed 103 kg with a BMI of 33, indicating the highest level of obesity. Doctor Lam Van Hoang, Head of the Endocrinology - Diabetes Department, stated that the patient's visceral fat index was double the safe threshold, accompanied by elevated liver enzymes and joint pain due to gout.

According to Doctor Hoang, Kien's two-year keto diet was ineffective because this diet severely restricts carbohydrate intake, limiting it to under 50 g daily. The ketogenic (keto) diet is characterized by very low carbohydrate consumption (sugars and starches) and high fat intake. Carbohydrates are replaced by fats and proteins, making fat the body's primary energy source. A keto diet typically provides 70% of calories from fats, 20% from protein, and 10% from carbohydrates.

When carbohydrate intake is reduced, the body enters a state called ketosis, burning fat instead of sugar or glucose from carbohydrates. While this diet often aids in weight loss and blood sugar reduction, these effects are typically temporary. In the long term, weight often returns, tending to increase beyond previous levels.

Furthermore, a long-term keto diet can lead to vitamin and mineral deficiencies, increasing the risk of cardiovascular diseases, cancer, elevated uric acid levels, hypocalcemia, and osteoporosis. A diet rich in protein and fats is unsuitable for individuals with impaired liver or kidney function and can negatively impact Kien's gout condition.

A nutritionist guided Kien to change his eating habits, focusing on a nutritionally balanced diet without the severe restrictions he previously followed. He was prescribed medication to suppress hunger and cravings while continuing to play soccer weekly.

After one month, Kien lost nearly 5 kg and a total of 14 kg over three months, experiencing reduced knee joint pain.

For sustainable weight loss, Doctor Hoang recommends gradually reducing calorie intake, emphasizing that individuals should not completely eliminate carbohydrates or other essential nutrients. He advises seeking medical consultation for a personalized weight loss plan.
