Around 3:12 p.m. on 20/1, a patrol team from the Quang Tri Provincial Police's Traffic Police Department was on duty on Ho Chi Minh road. The team included Captain Nguyen Anh Tuan (team leader), along with Major Ton That Cuong, Major Nguyen Khanh Long, Senior Lieutenant Phan Lam Phuong, and Senior Lieutenant Pham Hung Sinh.

Upon reaching Tan Ap hamlet in Tuyen Son commune, Senior Lieutenant Phan Lam Phuong signaled a motorbike to stop. The motorbike, driven by Hoang Gia Bao, was carrying Phan Trong Bao (17, from Phuc Trach commune, Ha Tinh province) and Nguyen Kim Chi (16, from Tuyen Son commune), and was traveling in the North-South direction. However, the motorbike crashed directly into Senior Lieutenant Phuong, throwing him approximately 5 meters. He then slid along the road, and the motorbike fell onto the shoulder.

As a result, Senior Lieutenant Phuong suffered a fractured left femur and pelvis. He received first aid at Huong Khe Hospital in Ha Tinh province before being transferred to Hue Central Hospital. The three individuals on the motorbike sustained minor scratches.

Tuyen Son Commune Police, in coordination with traffic police forces, took the victim for emergency treatment, secured the scene, and directed traffic. They also reported the incident to the Provincial Police for resolution.
