Is a decrease in morning erections a cause for concern?

Previously, I would have morning erections every day, which would subside after urination. However, in recent months, this has decreased, sometimes not occurring for an entire week. Is this something to worry about? (Ngoc Son, 44, Hanoi)

Response:

Morning erections are a normal phenomenon in men and are considered a sign of good physiological health. During sleep, the body undergoes several natural physiological changes that individuals may not consciously perceive. This is linked to sleep cycles, blood circulation, and hormone levels in the body.

Testosterone is a hormone associated with male libido and physiological function. Testosterone levels are typically highest around 5-8 h, immediately after waking, and lowest between 18-23 h. Early morning testosterone concentrations can be two to three times higher than in the late afternoon or evening. This difference is more pronounced in younger men and gradually diminishes with age due to the natural decline in testosterone levels. During the rapid eye movement (REM) sleep stage, the final phase of each sleep cycle, the brain-pituitary-testicular axis stimulates testosterone production. This allows the hormone to accumulate and peak in the early morning, which is the primary reason men experience this natural physiological phenomenon upon waking.

This early morning phenomenon is also influenced by stress, pressure, insomnia, and certain medications. Medical conditions such as atherosclerosis, high blood pressure, cardiovascular disease, and diabetes can impair blood flow to the genital area, leading to erectile dysfunction or the absence of morning erections.

A reduction or complete absence of morning erections is a sign that warrants attention. If the frequency decreases, men should adjust their lifestyle by ensuring adequate sleep, regular exercise, a balanced diet, and stress management. These steps support stable hormone levels and improve overall health. Individuals may also consider supplementing with natural nutrients such as eurycoma longifolia, maritime pine, or oysters to aid hormonal balance and circulation, but it is advisable to consult a doctor before use.

If the situation does not improve, or if accompanied by other signs of sexual dysfunction such as erectile dysfunction, premature ejaculation, or decreased libido, a medical consultation is recommended to assess the cause and determine appropriate treatment.
