Twin birth with two different presentations, one in an intact amniotic sac

Twin babies were born with different presentations, with the girl in a head-down position while her brother was breech, feet-first, and still in his intact amniotic sac, crying out at exactly 9:09 on 9/9.

A rare and unique birth occurred at Viet Tiep Friendship Hospital's Obstetrics Department, where twin babies were delivered with distinct presentations. The mother, who had a history of two previous C-sections, underwent another C-section on 9/9 despite experiencing natural labor.

The baby girl was born first in a favorable head-down presentation. Her brother, however, was in a breech presentation, feet-first, and remained entirely within his intact amniotic sac. Doctor Le Anh Hung, Head of the Obstetrics Department, carefully opened the amniotic sac. The baby boy cried out at exactly 9:09 on 9/9, weighing 2,9 kg. He also had his umbilical cord wrapped two times around his neck.

Hospital representatives described the birth as "both special and adorable." According to Doctor Hung, such cases are uncommon. While a breech presentation is not rare, it is considered an abnormal fetal position that carries a high risk of preterm birth and can complicate the delivery process. Obstetricians must carefully evaluate and select the delivery method and timing based on various factors to ensure the safety of both mother and child.

The umbilical cord, the lifeline connecting mother and child, typically measures 50-60 cm in length. An umbilical cord wrapped around the neck can occur due to excessive fetal movement, especially in the early months. Other factors increasing this risk include an abnormally long cord, polyhydramnios (excess amniotic fluid), or multiple pregnancies. Immediately after the baby was delivered, the medical team carefully unwrapped the cord, ensuring the baby's safety.

Typically, the amniotic sac ruptures due to uterine contractions during labor or medical intervention before birth. When a baby is born with the amniotic sac still intact, it is commonly known as "born in the caul" (de boc dieu), which is considered a sign of good fortune in folklore. Such children are believed to experience many advantages in life. A boy-girl twin pair is affectionately called a "dragon-phoenix pair" (cap long phuong) in Vietnamese tradition.

Twin pregnancies, especially with a mother's history of two previous C-sections, require closer monitoring. Physicians must simultaneously assess the condition of both fetuses and consider all medical factors during monitoring and management. On 10/9, a hospital representative confirmed that both mother and babies are healthy and recovering well.
