Why tonsillitis intensifies with recurring sinusitis

I have chronic sinusitis, and each time it recurs, my tonsils also become inflamed and painful. Why does this happen, and how is it treated? (Dung Nguyen, 45, Ho Chi Minh City)

The ear, nose, and throat form an interconnected system. When sinusitis recurs, pus from the sinuses drains down the throat, irritating the pharyngeal and tonsillar mucous membranes and worsening symptoms. Conversely, frequently inflamed tonsils can also be a source of infection affecting the sinuses, contributing to prolonged throat and nasal infections.

Chronic sinusitis typically lasts over three months, leading to anatomical changes within the sinuses. During this time, blocked sinus openings prevent fluid drainage. Concurrently, the sinus lining begins to swell and degenerate, often forming polyps.

For an accurate diagnosis and appropriate treatment, you should undergo nasal and sinus endoscopy. A CT scan or MRI may also be necessary for a comprehensive assessment of your sinonasal anatomy.

When chronic sinusitis does not respond to medical treatment, surgical intervention is often indicated to clear the infection and restore sinus drainage. After surgery, post-nasal drip improves, reducing irritants that cause pharyngitis and tonsillitis. However, this does not mean tonsillitis will be completely cured.

If your tonsils are chronically inflamed, hypertrophied, or have multiple recurrent infections, you may still experience tonsillitis even after your sinus conditions have resolved. In such cases, the doctor will assess the tonsillitis's severity to determine the appropriate treatment. Options include medical management or tonsillectomy if indicated. Therefore, treating sinusitis can help improve and reduce the frequency of tonsillitis, but it does not entirely replace treatment for the tonsil condition itself.

In some cases, patients may undergo "2-in-1" surgery, combining tonsil and sinus surgery to comprehensively address both conditions and effectively limit recurrent infections. However, this indication requires a thorough assessment of the patient's condition and the doctor's careful consideration.
