Ms. Truc visited Tam Anh General Hospital in TP HCM for examination, weighing 74 kg. She presented with a distended lower abdomen and persistent, unexplained pain. Imaging results revealed a large ovarian cyst, approximately 5 cm, on her right side, along with grade two fatty liver.
Doctor Lam Van Hoang, Head of the Department of Endocrinology - Diabetes, explained that polycystic ovaries are a primary cause of rapid weight gain, leading to grade two obesity. He noted the reciprocal relationship between polycystic ovaries and obesity. Hormonal imbalances during perimenopause can disrupt egg development and ovulation, preventing follicles from regressing properly and instead forming ovarian cysts. Common symptoms include persistent abdominal pain, bloating, and frequent urination.

Polycystic ovaries directly impair ovarian function, hindering the body's ability to properly absorb insulin, which results in insulin resistance. Consequently, sugar is not utilized by cells for energy but instead accumulates as excess fat, impeding fat breakdown. Additionally, insulin resistance promotes an increase in androgen, a male hormone, leading to rapid weight gain, particularly around the abdomen.
The doctor designed a weight loss regimen for Ms. Truc, incorporating appetite suppressants, calorie restriction, and medication to prevent diabetes. Concurrently, her diet was adjusted to emphasize vegetables and fruits, providing essential vitamins A and C to support cell activity and help prevent cyst progression. Adequate hydration and limiting foods high in protein and fat also contributed to stabilizing her endocrine system and improving metabolism.
After three months of treatment, Ms. Truc lost 10 kg. Her abdominal pain, bloating, and hot flashes significantly reduced. She continues regular follow-up appointments for ongoing monitoring to detect any unusual changes promptly.
