Dr. Duong Quang Huy, Head of the Andrology Unit at the Center for Reproductive Support, Tam Anh General Hospital - District 8, reported that Minh had a congenital inguinal hernia, measuring 10-12 cm. This mass compressed his scrotal-inguinal region, causing discomfort, swelling, and pain, especially when coughing or lifting heavy objects. It also displaced his penis.
The inguinal hernia also elevated scrotal temperature, impairing sperm quality. Furthermore, it compressed blood flow, causing testicular atrophy and difficulty conceiving. Minh's two in vitro fertilization (IVF) attempts failed due to this poor sperm quality.
For this procedure, Dr. Huy employed the Bassini technique. This method utilized Minh's own muscle layers and ligaments to close the inguinal floor. This approach avoids artificial mesh, which can lead to inflammation, adhesions, or vas deferens blockage, impacting reproductive health.
During the surgery, the doctor carefully dissected the vas deferens and testicular blood vessels. Inflamed, fibrotic omental masses were removed, and the remaining tissues were returned to the abdomen. The inguinal wall was then reconstructed using Minh's own muscle, and the penis and testicles were repositioned. Doctors anticipate sperm quality will improve within 3-6 months, potentially allowing Minh to conceive naturally. A third IVF attempt would also have a higher chance of success.

An inguinal hernia occurs when abdominal organs, such as intestines or omentum, protrude through the abdominal wall into the groin, creating a bulge. Early detection and treatment are crucial to prevent complications. A strangulated inguinal hernia, where the bowel becomes trapped, can lead to necrosis and be life-threatening. Additionally, if the hernia compresses testicular blood vessels and the vas deferens, men may experience reduced sperm quality and infertility, as seen in Minh's situation.
Dr. Huy advises men experiencing symptoms like unusually enlarged testicles, noticeable testicular displacement, pain, itching, or low ejaculation volume to consult a specialist. Early diagnosis and appropriate treatment are essential to preserve reproductive function.
