How to sleep when experiencing acid reflux?

I often experience nighttime gastroesophageal reflux. What sleeping position should I adopt to reduce these reflux episodes? asks Le from Dong Nai province.

Sleeping position significantly influences acid reflux symptoms. The lower esophageal sphincter (LES) naturally functions to prevent stomach acid from flowing back into the esophagus. However, when lying down, subtle changes in gravity can encourage acid to move upwards.

Sleeping on your left side can alleviate reflux symptoms. In this position, the stomach and pyloric valve are lower than the esophagus, making it harder for acid to flow back up. Left-side sleeping also promotes faster stomach emptying, reducing pressure and improving nighttime heartburn. Conversely, sleeping on your right side can increase reflux frequency because the stomach is positioned higher than the esophagus, allowing acid to move upwards more easily.

Elevating your head and upper body while sleeping on your back is another suitable option. Raising the head of your bed by about 15-20 cm uses gravity to keep acid in the stomach, thus limiting reflux. Avoid using multiple pillows for elevation, as this is often ineffective and can cause neck strain and shoulder pain. To aid digestion, sit upright or recline slightly after meals, and avoid lying down immediately after eating. Instead, wait two to three hours after a meal before sleeping to allow proper digestion and reduce reflux risk. Additionally, maintain healthy lifestyle habits: limit alcohol, coffee, and medications, and avoid fatty, spicy foods, as these can prolong digestion and increase the risk of reflux.

Seek medical attention if reflux symptoms persist or worsen despite lifestyle changes. If reflux symptoms such as heartburn, chest pain, difficulty swallowing, or a persistent nighttime cough occur frequently (two times a week or more) or do not improve after lifestyle adjustments, consult a gastroenterologist. Untreated gastroesophageal reflux disease can lead to serious complications including esophagitis, esophageal ulcers, esophageal stricture, or Barrett's esophagus.
